Overview: <p>This Care Manager will work full time, Monday through Friday 8:00AM - 430 PM and be centered in our new Med Surg Unit.</p> Responsibilities: The Care Manager is responsible for care coordination, progression of care, and proactive discharge planning and is accountable for expediting the timely and safe discharge for all patients in their case load.
