<p> <strong>Primary Vascular Care</strong> is a specialized vascular group dedicated exclusively to serving the long-term care space. We bring a clinic-like experience-providing advanced vascular care-directly into nursing homes. </p><ul><li><p> Personalized bedside care minimizes the need for patients to leave their familiar environment. </p></li><li><p> From routine vascular assessments to advanced procedures, our team ensures comprehensive, high-quality care for all our patients. </p></li></ul><p> Primary Vascular Care is looking for a <strong>Nurse Practitioner (NP) or Physician Assistant (PA)</strong> to perform on-site clinical assessments to determine appropriate patient treatment plans of residents of client facilities. This position reports to the Lead Nurse Practitioner. </p><p> <strong>The NP/PA will work in a mobile practice, primarily serving a long-term care setting.</strong> </p><p> <strong>Must be able to travel to Queens and the Bronx.</strong> </p><p></p><p> <strong><u>ESSENTIAL FUNCTIONS:</u></strong> </p><ul><li><p> Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by the completion of a comprehensive assessment form </p></li><li><p> Functions independently to perform appropriate history and physical for complex acute, critical, and chronically ill perioperative patients </p></li><li><p> Orders and reviews diagnostic and therapeutic tests relative to patient's age-specific needs </p></li><li><p> Assists with the provision of care in accordance with facility, state, and federal regulations </p></li><li><p> Monitors the effectiveness of interventions </p></li><li><p> Maintains relationships with all appropriate contacts in facilities </p></li><li><p> Educates family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care </p></li><li><p> Assists with the development and maintenance of physician relations, as assigned </p></li><li><p> Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner </p></li></ul><p></p><p> <strong><u>QUALIFICATIONS:</u></strong> </p><ul><li><p> Bachelor's Degree required </p></li><li><p> Degree from an accredited Nurse Practitioner or Physician Assistant Program required </p></li><li><p> Current NP License required </p></li><li><p> BLS - Basic Life Support through American Heart Association required </p></li><li><p> ACLS - Advanced Cardiac Life Support through American Heart Association preferred </p></li><li><p> Current Infection Control Certification required </p></li><li><p> Board Certified/Board Eligible required </p></li><li><p> Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella required </p></li><li><p> COVID and Flu Vaccine preferred </p></li><li><p> 1 year of NP experience preferred </p></li><li><p> Reliable transportation (personal vehicle) required </p></li></ul><p> <br> </p><img src="https://www.jobg8.com/Tracking.aspx?hpOEVJHkfW9YHUCDMCZgLWc7Rwgmb1DQr" width="0" height="0" />
