<strong>Registered Dietitian - PACE Program</strong> <p><strong>Location:</strong> Evansville, IN<br /> <strong>Job Type:</strong> Full-Time<br /> <strong>Salary Range: </strong>$70k - $73k and comprehensive benefits</p> <strong>Position Summary</strong> <p>We are seeking a <strong>Registered Dietitian</strong> to join a mission-driven PACE (Program of All-Inclusive Care for the Elderly) organization. This role is responsible for assessing participants' nutritional status, developing individualized nutrition interventions, and collaborating with the Interdisciplinary Team (IDT) to ensure comprehensive, participant-centered care.</p> <p>The Registered Dietitian plays a critical role in supporting frail and elderly participants through nutrition counseling, care planning, and ongoing monitoring to promote health, independence, and quality of life.</p> <strong>Key Responsibilities</strong> <ul> <li> <p>Conduct comprehensive nutritional assessments at enrollment and periodic reassessments per PACE regulations</p> </li> <li> <p>Develop and update individualized nutrition care plans in collaboration with the interdisciplinary team</p> </li> <li> <p>Provide nutrition counseling and education to participants, caregivers, and families</p> </li> <li> <p>Monitor weight, hydration, intake, swallowing/chewing issues, mood changes, skin integrity, and drug-nutrient interactions</p> </li> <li> <p>Recommend and coordinate nutrition-related interventions such as home-delivered meals and supplements</p> </li> <li> <p>Communicate participant nutrition needs and recommendations to the IDT</p> </li> <li> <p>Participate in interdisciplinary team meetings, family meetings, and care conferences</p> </li> <li> <p>Incorporate cultural and ethnic preferences into individualized nutrition plans</p> </li> <li> <p>Provide education and guidance to healthcare professionals and center staff</p> </li> <li> <p>Participate in quality improvement initiatives and training programs</p> </li> <li> <p>Maintain accurate, comprehensive documentation and follow infection control and safety protocols</p> </li> <li> <p>Perform additional duties as assigned, including Food Service Supervisor responsibilities when applicable</p> </li> </ul> <strong>Qualifications & Experience</strong> <ul> <li> <p>Bachelor's degree in nutrition, dietetics, or related field from an accredited college</p> </li> <li> <p>Registered Dietitian credential (passing the Commission on Dietetic Registration exam)</p> </li> <li> <p>Minimum 1 year experience with elderly or frail populations (training provided if not present)</p> </li> <li> <p>Minimum 2 years clinical nutrition experience preferred</p> </li> <li> <p>ServSafe certification preferred</p> </li> </ul> <strong>Skills & Competencies</strong> <ul> <li> <p>Strong clinical assessment and care planning skills</p> </li> <li> <p>Knowledge of geriatric nutrition, chronic disease management, and drug-nutrient interactions</p> </li> <li> <p>Excellent communication and counseling skills</p> </li> <li> <p>Ability to collaborate within an interdisciplinary care model</p> </li> <li> <p>Strong documentation and organizational skills</p> </li> <li> <p>Cultural competency and patient-centered approach to care</p> </li> </ul> <strong>Why Join a PACE Program?</strong> <p>PACE programs provide comprehensive, coordinated care to older adults, helping them remain independent and avoid institutional care.
