Department: LR-Home Health RN<br><br>Shift: Day<br><br>Working Hours: 8-4:30 pm<br><br>Summary:<br>Baptist Health Home Health/Hospice RN Case Manager provides, manages and coordinates care and services for an assigned caseload of patients.<br><br>Other information:<br>Education required: Graduation from an approved program of Nursing.<br>Required: Current Arkansas RN License.<br>Required: Current Arkansas Driver's License.<br>Preferred: Minimum 1 year of nursing experience.
