In a variety of locations and community-based settings, we offer a range of programs, including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members and Veterans. <br/><br/><p> <strong>Pediatrics Program</strong> </p> <p> <strong>Monday - Friday Full-time with benefits </strong> </p> <p> <strong>Salary: $75,000 - $95,000</strong> </p> <p> <strong>Sign on bonus up to $5,000</strong> </p> <p> <strong>Relocation assistance $5,000</strong> </p> <p><strong>Employer paid annual PT licensure</strong></p> <p><strong>Employer CEU allowance up to $5000</strong></p> <p><strong>Employer paid work supplies</strong></p> <p><strong>Employer paid Crisis Prevention Intervention (NCI) Certification</strong></p> <p><strong>Employer paid Certified Brain Injury Specialist training-annual rotation</strong></p> <p><strong>Employer paid CPR Certification</strong></p> <p><strong>Employer paid lunch</strong></p> <p> </p> SUMMARY <p> Provides medically prescribed physical therapy treatment for patients suffering from injuries, deconditioning, or muscle, nerve, joint, and bone diseases to restore function, relieve pain, and prevent disability. <br><br> </p> <p> <strong>ESSENTIAL JOB FUNCTIONS</strong> </p> <p> To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below: </p> <ul> <li> Evaluates program participants through appropriate evaluation procedures and develops written treatment programs for each participant. </li> <li> Administers appropriate treatment programs and communicates program participant needs to appropriate staff. Provides needed re-evaluation and programmatic changes as necessary. </li> <li> Maintains timely and appropriate program participant/administrative and department documentation. </li> <li> Ensures adherence to accreditation standards, licensure standards, professional code of ethics, and confidentiality. </li> <li> Attends programmatic meetings on request for team reports, policy setting, exchange of information, etc.; attends other appropriate meetings/conferences as needed. </li> <li> Attends clinical meetings to exchange, discuss, and evaluate program participant treatment programs. </li> <li> Serves on rehabilitation treatment team(s). Communicates with physicians, staff, professionals/paraprofessionals, consultants, families, and community resources as needed to ensure effectiveness of physical therapy program. </li> <li> Plans and implements in-service programs for staff and other groups as requested/assigned by supervisor. </li> <li> Participates in orientation programs for new staff, program participants, and PT/PTA students. </li> <li> Writes service summaries as requested by case manager/program director and maintains daily billing summaries and turns them into the appropriate staff. </li> <li> Performs other related duties and activities as required. </li> </ul> <p> </p> <p> <strong> </strong> </p> <p> <strong><em>Education and Experience: </em></strong> </p> <ul> <li> Degree in Physical Therapy </li> </ul> <p> <strong><em>Certificates, Licenses, and Registrations: </em></strong> </p> <ul> <li> State licensure in physical therapy. </li> </ul> <p> </p> <p> <strong>AMERICANS WITH DISABILITIES ACT STATEMENT </strong> </p> <p> External and internal applicants, as well as position incumbents who become disabled, must be able to perform the essential job functions either unaided or with assistance of a reasonable accommodations to be determined on a case by case basis. </p><br/><br/> <p> Sevita is a leading provider of home and community-based specialized health care.
