<p><strong>Physical Therapist (PT) - Skilled Nursing Facility</strong></p> <p>RCM Health Care Services is seeking dedicated and compassionate Physical Therapists (PT) to join our team to support our Skilled Nursing Facilities (SNF). The ideal candidate will provide high-quality rehabilitative care to residents, helping them regain mobility, strength, and independence. This role involves assessing patient needs, developing individualized treatment plans, and working collaboratively with an interdisciplinary healthcare team.</p> <p><strong>Full-time 13-Week Positions (chance for extension)!</strong></p> <p><strong>Physical Therapist Key Responsibilities:</strong></p> <ul> <li>Conduct comprehensive evaluations to assess patient needs <li>Develop and implement individualized treatment plans <li>Provide therapy to enhance mobility, strength, and functional abilities <li>Collaborate with a multidisciplinary team to ensure holistic care <li>Maintain accurate and timely documentation of patient progress <li>Educate patients and families about therapy goals and techniques </ul> <p><strong>Physical Therapist Qualifications:</strong></p> <ul> <li><strong>Education:</strong> Doctorate (DPT) or Master's Degree in Physical Therapy from an accredited program. <li><strong>Licensure:</strong> Active <strong>Physical Therapist license</strong> in New York State or eligibility to obtain one. <li><strong>Experience:</strong> Prior experience in a skilled nursing facility or geriatric rehabilitation preferred but not required. <li><strong>Skills:</strong> Strong communication, problem-solving, and patient-care skills.
