<p><strong>Physical Therapist (PT) - Skilled Nursing Facility</strong></p> <p>RCM Health Care Services is seeking dedicated and compassionate Physical Therapists (PT) to join our team to support our Skilled Nursing Facilities (SNF). The ideal candidate will provide high-quality rehabilitative care to residents, helping them regain mobility, strength, and independence. This role involves assessing patient needs, developing individualized treatment plans, and working collaboratively with an interdisciplinary healthcare team.</p> <p><strong>Full-Time / Part-Time / PRN</strong></p> <p><strong>Physical Therapist Key Responsibilities:</strong></p> <ul> <li>Evaluate residents' physical conditions and functional abilities through assessments.</li> <li>Develop and implement personalized treatment plans to improve mobility, strength, balance, and overall function.</li> <li>Provide hands-on therapy techniques, exercises, and interventions tailored to residents' needs.</li> <li>Educate residents, caregivers, and families on techniques to enhance recovery and prevent further injury.</li> <li>Monitor patient progress, adjust treatment plans as necessary, and document therapy sessions accurately.</li> <li>Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to optimize patient outcomes.</li> <li>Ensure compliance with state and federal regulations, as well as facility policies and procedures.</li> <li>Maintain accurate and timely records in the electronic medical record system.</li> <li>Participate in discharge planning and recommend appropriate adaptive equipment or continued therapy services.</li> </ul> <p><strong>Physical Therapist Qualifications:</strong></p> <ul> <li><strong>Education:</strong> Doctorate (DPT) or Master's Degree in Physical Therapy from an accredited program.</li> <li><strong>Licensure:</strong> Active <strong>Physical Therapist license</strong> in New York State or eligibility to obtain one.</li> <li><strong>Experience:</strong> Prior experience in a skilled nursing facility or geriatric rehabilitation preferred but not required.</li> <li><strong>Skills:</strong> Strong communication, problem-solving, and patient-care skills.
