Our Physical therapist work as a team with physical therapy assistants to treat people of all ages and abilities. In this position with Presbyterian's Outpatient rehabilitation department gives clinicians the opportunity to treat a variety of patients and the ability to work with a group of therapist that have a range in specialties. The Outpatient rehabilitation team is collaborative and patient-focused. We place a strong emphasis on providing orientation and mentorship in order to ensure the well-being and success of our new team members. Join a team committed to improving the health of our communities.</p><p></p><p>Additional Job Duties:</p><p>Evaluates patients, develops <b>plans </b>and goals, and implements treatment programs for improvement of physical or d<b>evelopmental dysfunction </b>and independence in activities of daily living.<br /><br />Continuously reassesses <b>treatment plans </b>and goals.<br /><br />Educates and instructs regarding home programs and <b>preventive </b>measures as appropriate to patient, family, and others.<br /><br />Prepares written documentation as required by the profession and the department in a timely manner.<br /><br />Communicates with patient, family, physician, and other healthcare providers regarding patient care.<br /><br />Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.<br /><br />Participates in educational programs and inservices and other meetings as required.
