Join us in our mission to enhance the well-being of our patients and residents while making a meaningful impact in the communities we serve. <em><strong>The hours for this position are Monday through Friday 10:30 am to 7pm</strong></em> Responsibilities: The RN Registered Nurse Admissions and Discharges is primarily responsible for the following job duties:<br /><ul><li><p>Assists with ensuring rooms are ready for new admissions</p></li><li><p>Greet new patients warmly and orient them to their new environment</p></li><li><p>Complete admission assessments and documentation, including initiation of the baseline care plans</p></li><li><p>Communicate patient status with interdisciplinary team</p></li><li><p>Complete medication reconciliation and order entry</p></li><li><p>Obtain consents if needed</p></li><li><p>Assist with patient and caregiver education</p></li><li><p>Ensure discharge orders are accurate</p></li><li><p>Complete discharge documentation</p></li><li><p>Complete comprehensive discharge education on post-discharge instructions, including wound care, or new medications.</p></li><li><p>Coordinate post discharge transition of care appointments</p></li><li><p>Complete post discharge follow up calls and documentation</p></li><li><p>Help identify patients at high risk for rehospitalization and implement early interventions</p></li><li><p>Work collaboratively with interdisciplinary team members to ensure smooth transition</p></li><li><p>Complete admission paperwork with the resident/resident representative (MA 401)</p></li><li><p>Assist with other duties as assigned</p></li></ul> Qualifications: Must be a graduate of an accredited school of nursing, college, or university; BSN preferred.
