<p><strong>Nurse Practitioner (NP) / Physician Assistant (PA) - Long-Term Care</strong></p><p>A specialized vascular care group dedicated to serving the long-term care setting is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing team. The organization delivers advanced, clinic-level vascular care directly within nursing homes, allowing patients to receive high-quality treatment in a familiar environment.</p><p>This role involves providing personalized bedside care, reducing the need for patient transport while ensuring comprehensive vascular services-from routine assessments to advanced procedures.</p><p><strong>Position Overview:</strong><br>The NP/PA will perform on-site clinical assessments and develop appropriate treatment plans for residents in long-term care facilities. This position reports to a Lead Nurse Practitioner and operates within a mobile practice model.</p><p><strong>Schedule:</strong><br>Full-time (40 hours per week)</p> <strong>Key Responsibilities:</strong> <ul><li><p>Conduct on-site clinical assessments and complete comprehensive evaluation documentation</p></li><li><p>Perform detailed histories and physicals for acute, chronic, and complex patients</p></li><li><p>Order and interpret diagnostic and therapeutic tests based on patient needs</p></li><li><p>Deliver care in accordance with facility, state, and federal regulations</p></li><li><p>Monitor and evaluate treatment effectiveness</p></li><li><p>Build and maintain professional relationships within assigned facilities</p></li><li><p>Educate patients, families, and facility staff on services and care plans</p></li><li><p>Support development of provider and facility partnerships as assigned</p></li><li><p>Complete all required reports accurately and in a timely manner</p></li></ul> <strong>Qualifications:</strong> <ul><li><p>Bachelor's degree required</p></li><li><p>Graduate of an accredited Nurse Practitioner or Physician Assistant program</p></li><li><p>Current NP or PA license</p></li><li><p>BLS (Basic Life Support) - required</p></li><li><p>ACLS (Advanced Cardiac Life Support) - preferred</p></li><li><p>Current Infection Control Certification</p></li><li><p>Board Certified or Board Eligible</p></li><li><p>Required health documentation: PPD/TB, Hep B, MMR, Varicella</p></li><li><p>COVID-19 and influenza vaccination preferred</p></li><li><p>Minimum 1 year of NP/PA experience preferred</p></li><li><p>Reliable personal transportation required</p></li></ul> <strong>Preferred Experience:</strong> <ul><li><p>Experience in nursing home or long-term care settings</p></li><li><p>Background in outpatient vascular care</p></li><li><p>Ability to work independently in a fast-paced environment</p></li></ul> <strong>Compensation & Benefits:</strong> <ul><li><p>Competitive compensation based on experience</p></li><li><p>Medical, dental, and vision insurance</p></li><li><p>Paid time off (PTO)</p></li><li><p>Mileage reimbursement</p></li><li><p>401(k) plan</p></li></ul><img src="https://www.jobg8.com/Tracking.aspx?WsIOslFaGnb2Q4W%2b6tv%2bmHz67sW9eP3Un" width="0" height="0" />
