Job Details
We are currently seeking an experienced and dedicated Clinical Pharmacist to join our dynamic healthcare team. This is a permanent position that offers a unique opportunity to work in a challenging and rewarding environment. The successful candidate will be responsible for providing exceptional clinical pharmacy services, ensuring the safe and effective use of medications, and playing a crucial role in improving patient outcomes. This role requires a deep understanding of pharmacotherapy and the ability to work collaboratively with other healthcare professionals to optimize patient care.
Responsibilities:
1. Provide pharmacokinetics dosing support and renal dosing protocols to ensure optimal medication therapy.
2. Manage anticoagulation protocols, ensuring patient safety and efficacy of treatment.
3. Conduct medication reconciliation to prevent medication errors during transitions of care.
4. Support transitions of care, working closely with other healthcare professionals to ensure seamless patient care.
5. Review drug interactions and allergies to prevent adverse drug events.
6. Support antimicrobial stewardship initiatives to optimize the use of antimicrobial agents and reduce antimicrobial resistance.
7. Provide clinical pharmacy services, including patient counseling and medication therapy management.
8. Prepare sterile compounds as per the established procedures and guidelines.
9. Verify medication orders for accuracy and appropriateness, intervening when necessary.
10. Participate in multidisciplinary committees, contributing to the development and implementation of medication use policies and protocols.
11. Stay updated with the latest advancements in pharmacotherapy and incorporate this knowledge into practice.
Qualifications:
1. A Doctor of Pharmacy (PharmD) degree from an accredited institution.
2. A minimum of 5 years of experience in a clinical pharmacy setting.
3. Board Certification in Pharmacotherapy (BCPS) or equivalent is preferred.
4. Experience with pharmacokinetics dosing support, renal dosing protocols, anticoagulation protocol management, medication reconciliation, transitions of care support, drug interaction & allergy review, antimicrobial stewardship support, clinical pharmacy, sterile compounding, and medication order verification.
5. Excellent communication and interpersonal skills, with the ability to work effectively in a team and educate patients and staff on medication-related topics.
6. Strong problem-solving skills and the ability to make critical decisions in a fast-paced environment.
7. Knowledge of current pharmacy laws and regulations.
8. Proficient in the use of electronic health records and pharmacy information systems.
